- Application Form: Application form is filled and signed by the applicant
- Born at Home: in case you born at home you have to provide an affidavit from parents with CNIC copies of 2 witnesses
Procedure to obtain Birth Certificate:
Applicant will first fill the application form and submit it with the supporting documents in the issuing department. In general areas and big cities NADRA birth certificates can be obtained from the local Union Councils but in small cities it can be obtain from TMAs, in Cantonment areas from Cantonment Board Offices and in Islamabad from CDA.
After the submission of the required documents, issuing Union council or the NADRA authorities will verify all submitted documents and after their approval Birth Certificate is issued.
